First caveat is you must be jailbroken.

Jailbroken toggle for sbsettings is called "Bluetooth mono" and it pushes all the audio out to the stereo....but sounds horrendous. Might be useful if you just wanted your gps commands to come through your stereo though...?

After posting I looked for one, and there's a "BT Mono" app, but I can't get it to work. Ah well, if it sounds bad then no loss...

The only nice thing I found was that it will interrupt your audio feed to give you navigation prompts and then resume whatever you were listening to.
